There are two recognized amorphisms of the areola
(a) Areola too capacious
It's undeniable that some women have big areola than normal. When you say broad it simply means it's unaccordant to the size of the nipple or simply means bigger than the usual.
To improve them, you will have to go through areola reduction procedure. The surgery will reduce areola's diameter. The subject will be injected with local anaesthetic. A desirable female breast is about 4.5 cm in diameter. However if the patient is full breasted , the areola will be reduced relative to the size of her nipple. A border of areola will be taken awayleaving only the proportioned skin. The residual skin will be snagged to the new areola size and absorbable sutures will be used so they don't actually need to be replaced, unlike the regular way of binding skins through stitching.
However, you will have a scar around the outside ring of your areola, which is quiet evident. The pale scar around areola however, can be treated with a medical-grade tattoo, which basically fills in the color of the scar to make it look natural. And you may further discuss that with your attending plastic/cosmetic surgeon.
(b) Puffiness of the Areola
Some women have a somewhat puffy areola. This is because of an extra sub adjacent breast tissue underneath the areola, pushing it forward that leads the areola to protrude. This is medically known as tuberous breast deformity.
To correct this condition, the tissue under the areola that is pushing the skin forward should be removed under local anaesthetics. You will need to make an opening around the nipple if it's puffy with large sized areola or a small opening at the base of the areola (at the junction of the brown areola and the skin) to remove the breast tissue if the areola is at it's normal size. This will set the areola tissue giving a much flatter appearance.
However, often times it can be combined with areola reduction surgery , if you have this lump of tissue under the areola, the lumpness may have pushed the areola to wide making the areola skin appear big after the removal of the tissue. Thus the areola reduction surgery maybe performed to make the areola proportioned to how big the nipple of the breast is. But if the areola is of it's normal size after the removal of the excess tissue underneath it, areoal reduction surgery is no longer needed.
